Objective and Management of OT II

Subject: Medical and Surgical Nursing II (Theory)

Overview

A scrub nurse does their duties in a sterile surgical setting. As he approaches the surgical station, he is "scrubbed in," donning sterile masks and attire. The operating area and the rest of the hospital are connected by a circulating nurse. She isn't dressed in scrubs. As the patient is taken into the operating room, a circulating nurse performs an initial examination of him and checks on his comfort. In OT, fire incidents occur frequently. As they must take care of diathermy machines, electrical wires, and other risks, OT nurses play a significant role in preventing such incidents. They should be aware of the sources of the hazards in order to assist in their prevention.

Responsibilities of scrub nurse, circulatory nurse, and recovery room

Scrub nurse

  • A scrub nurse does their duties in a sterile surgical setting. As he approaches the surgical station, he is "scrubbed in," donning sterile masks and attire.
  • The surgical doctor receives the surgical instruments and other materials from the scrub nurse.
  • A scrub nurse keeps the operating room clean and ensures that everything is kept sterile to lower the risk of contamination.
  • The care of the surgeon is also under the purview of scrub nurses. The scrub nurse is in charge of taking care of any issues, such as sweating or hazy glasses, so the surgeon can focus on her work uninterrupted.
  • The scrub nurse is also in charge of keeping an eye on the patient's wellbeing throughout the procedure. In order to alert the doctor, the scrub nurse must monitor the patient's vital signs.

Circulating Nurse

  • The operating area and the rest of the hospital are connected by a circulating nurse. She isn't dressed in scrubs.
  • As the patient is taken into the operating room, a circulating nurse performs an initial examination of him and checks on his comfort.
  • The scrub nurse and surgeon receive assistance from the circulating nurse as they tidy up and get ready for operation.
  • As needed, circulating nurses will hand supplies packages to the scrub nurse during the procedure.
  • A circulating nurse is responsible for getting things from outside the room or relaying messages from the surgeon to other staff members or family members.
  • The circulating nurse counts opened packages and used supplies after the procedure to ensure the totals are accurate. This is to make sure that during the procedure, no supplies were inadvertently left inside the patient.

Prevention of Accidents and Fire Hazards

In OT, fire incidents occur frequently. As they must take care of diathermy machines, electrical wires, and other hazards, OT nurses play a significant role in preventing such accidents. They should be aware of the causes of the hazards in order to assist in their prevention.

Three factors are responsible for fire in OT

  • Fuel: Fuel refers to combustible materials like ether, ethyl chloride, spirit, grease, and oil. These compounds can cause fire because they are flammable, burn when exposed to oxygen and nitrous oxide, and are flammable in general.
  • Gasses used in OT such as oxygen, air, nitrous oxide
  • Source of fire: There are two main sources of fire
    • Electricity and
    • Diathermy

Other sources of Fire

  • Electrical connections and wires that are leaking
  • Endoscopes, cauterie, lasers, and resectoscopes all emit heat. heat probe, fiber optic light, drill, organ beam, and OT light.
  • Rapidly opening compressed cylinders can produce heat up to 170°F.
  • Electrical equipment sparks: If there are sparks when turning on or off equipment, nearby flammable materials could catch fire.

Preventive measures

  • There ought to be fire-extinguishing equipment.
  • Antistatic flooring ought to be used in OT. There are particular toreros floor kinds with integrated steel wires and caps on black that stop the formation of static electricity.
  • Rubber should be constructed of antistatic rubber, as should stool wheels, shoe soles, and other items.
  • Rebreathing bags and tubes for anesthetic machines should be made of carbon-infused antistatic rubber. The word "antistatic" should be written and there should be a yellow marking.
  • Avoid wearing hosiery, nylon, or terylene clothing. In the OT room, only cotton clothing should be worn.
  • Use only equipment that cannot ignite nearby flammable materials if there are sparks when starting or shutting off the device.
  • The patient should not be near a monitor.
  • The foot pedal should be operated by a surgeon, who should also release it when not needed.
  • Thermolysis ought to sound an alarm.
  • Never use alcohol, grease, or oil on the oxygen cylinder.
  • Open the cylinder slowly and meticulously.
  • There ought to be a fire alarm.

Questions and Answers

Scrub nurse

  • A scrub nurse works in a sterile surgical area. Before approaching the surgical station, he "scrubs in," putting on sterile masks and clothing.
  • The scrub nurse hands the surgical tools and supplies to the doctor performing the procedure.
  • A scrub nurse is responsible for keeping the operating room clean and sterile in order to reduce the risk of contamination.
  • Scrub nurses are also in charge of the surgeon's care. If her glasses become foggy or she begins to sweat, the scrub nurse is in charge of taking care of those issues, allowing the surgeon to continue her work uninterrupted.
  • The scrub nurse is also in charge of monitoring the patient's health during surgery. This includes keeping track of the patient's vital signs and alerting the doctor if anything changes.
  • A circulating nurse is a liaison between the operating room and the rest of the hospital. She has not been scrubbed in.
  • As the patient is wheeled into the operating room, a circulating nurse assesses his condition and ensures that he is comfortable.
  • The circulating nurse also helps the surgeon and scrub nurse clean up and get ready for surgery.
  • During the procedure, circulating nurses pass supplies to the scrub nurse as needed.
  • A circulating nurse is responsible for getting something from outside the room or relaying a message from the surgeon to another member of staff or a family member.
  • The circulating nurse counts opened packages and used supplies after surgery to ensure that the numbers add up correctly. This is done to ensure that no supplies were accidentally left inside the patient during the surgery.

Preventive measures

  • There should be fire extinguisher devices available.
  • Antistatic flooring should be used in OT. There are special toreros types of floors with steel wires and capon black embedded to prevent static electricity generation.
  • Antistatic rubber should be used for trolley wheels, stool wheels, shoe soles, and other items.
  • Rebreathing bags and tubes on anesthetic machines should be made of antistatic, carbon-containing rubber. There should be yellow markings with the word "antistatic" written on them.
  • Avoid wearing nylon, terylene, or hosiery. Only cotton clothing should be worn in the OT room.
  • If there are sparks when starting or switching off equipment, the inflammable substances around it can catch fire.
  • The monitor should be kept at a safe distance from the patient.
  • The surgeon should operate the foot pedal and deactivate it when not needed.
  • Diathermy should be monitored.
  • No oil, grease, or alcohol should be used on the oxygen cylinder.
  • Slowly and carefully open the cylinder.
  • A fire alarm should be installed.
