Benzathine Penicillin, Benzyl Penicillin and Amoxycillin

Overview

Benzathine Penicillin

It is a penicillin with a long acting. Streptococcal, meningococcal, and gonococcal infections are its principal uses.

Indication:

  • Rheumatic fever
  • Rheumatic arthritis
  • Syphilis
  • Pyoderma

Dose:

  • For streptococcal infections: IM 12 lakh (900mg) single dose.
  • Prophylaxis of acute rheumatic fever: 12 lakh (900mg) IM every 3 - 4 weeks.
  • Syphilis: 24 lakh IM in 2 sites. If more than one year duration 24 lakh weekly for 3 weeks.

Nursing Consideration:

  • Before giving Penicillin, a skin test should be performed.
  • Course of medication even though symptoms go away.
  • To keep therapeutic serum drug levels, benzothine penicillin should be given continuously.
  • Before taking the first dose, collect a sample for the sensitivity test.
  • When administering doses repeatedly, rotate the injection site and perform a deep intramuscular injection in the upper outer quadrant of the buttock.
  • If your patient exhibits any hypersensitivity symptoms (such as fever, wheezing, skin rashes, generalized itching, or dyspnea), stop the medicine and call your doctor very away.

Benzyl Penicillin

It is a penicillin with a quick acting. Penicillin may also go by the names crystalline or aqueous.

Indication:

  • Respiratory tract infection.
  • Urinary tract infection.
  • Diphtheria, Tetanus, Syphilis,
  • Gonorrhoea, pyoderma, Otitismedia

Dose:

  • Adults: 5-10 lac units qid for 5-7 days.
  • Child: 15-30mg/kg/dose IV infusion over 15-30 minutes or IM. 60mg/kg/dose for severe sepsis and meningitis.

Nursing Consideration:

  • Before taking the first dose, collect a sample for the sensitivity test.
  • Make sure the patient is not allergic to penicillin before giving them any (a skin test or ID test is required). It is advisable to perform a drug sensitivity test prior to beginning medication.
  • If your patient exhibits symptoms of hypersensitivity, such as fever, wheezing, skin rashes, generalized itching, or dyspnea, stop the drug and let the doctor know.
  • The fact that Benzyl Penicillin is also referred to as Crystalline Penicillin should not mislead nurses (usually 10lac unit is available)
  • The patient should be instructed to take oral penicillin with a full glass of water at least an hour before or two hours after a meal.
  • It is advisable to provide this short-acting penicillin 4-6 times per day (Meningitis). As soon as mixing with diluents is finished, an IV injection should be administered.
  • A 10-15 minute IV infusion should be administered steadily.
  • To keep therapeutic serum drug levels, benzoyl penicillin should be given continuously.

Amoxycillin

It is a broad spectrum/extended spectrum aminopenicillin antibiotic.

Indication:

  • Respiratory tract infection: bronchitis, pneumonia
  • ENT infection: sinusitis, otitismedia, tonsillitis
  • Dental & upper respiratory procedure: Prevention of bacterial endocarditis
  • Tripal therapy: H.pylori eradication

Dose:

  • Adult: 250-500mg tid for 5-7 days
  • Child: 50mg / k * g / d * ay in three divided dose

Nursing Consideration:

  • Get a history to learn about any penicillin or cephalosporin usage in the past, as well as any responses.
  • Before administering the first dose of medicine, get samples for culture and sensitivity testing.
  • Intravenous solutions should be started gradually at initially while being closely monitored for sensitive response.
  • Give medication continuously to keep blood plasma levels stable. Never skip a dose.
  • If your patient exhibits any symptoms of hypersensitivity, such as fever, wheezing, skin rashes, generalized itching, or dyspnea, stop the medicine and call your doctor right once.
  • In order to rule out pseudomembranous colitis, closely monitor diarrhea.
  • While amoxicillin can be taken at any time, ampicillin should be taken before meals (either before, during or after meals.)
  • How to reconstitute amoxicillin or Ampicillin:
    • Fill the bottle with new water until it is about 1 cm below the fill line, then seal it and give it a good shake to reconstitute the suspension.
    • Once the foam has subsided, slowly fill the mark with fresh tap water.
    • Shake ferociously once more.
    • The now-useable suspension is white to slightly yellowish.
    • Before each withdrawal, give the bottle a good shake.
